Low testosterone gets thrown around a lot online, often alongside questionable supplements promising to "naturally boost T levels overnight." The real research on what actually affects testosterone is more specific — and more useful — than most of that marketing.

Low testosterone (low T) is typically associated with persistent fatigue, reduced strength and muscle mass, low libido, mood changes, and poor sleep quality. These symptoms overlap with a lot of other conditions, which is exactly why bloodwork — not guesswork — is the right first step if you suspect low T.

Sleep: A Direct, Measurable Cause

One of the clearest pieces of evidence in this area comes from a study published in JAMA by researchers Rachel Leproult and Eve Van Cauter in 2011. In the study, young healthy men who restricted their sleep to just 5 hours a night for one week saw a measurable 10–15% drop in daytime testosterone levels — and the effect grew larger the longer the sleep restriction continued. This is one of the most direct causal demonstrations linking sleep specifically to testosterone in the research literature.

Visceral Fat: More Important Than Overall Weight

A 2026 study in the Journal of the Endocrine Society, by Karl Friedl and Adam Potter, found that visceral fat — the fat stored deep around your abdominal organs — is more strongly linked to low testosterone than overall body mass index. Specifically, men with high visceral adipose tissue had 33–36% lower testosterone than men with low visceral fat, even within the same BMI category. A normal-weight man carrying excess visceral fat had testosterone levels comparable to an obese man with low visceral fat. The researchers describe low testosterone as "a phenotype of visceral fat biology" rather than a simple consequence of being overweight — meaning targeting visceral fat specifically, not just the scale number, is the more direct lever.
